Health Preparation for Sierra Leone

Sierra Leone requires more thorough health preparation than most travel destinations. This is not to alarm — hundreds of thousands visit safely every year — but to ensure you arrive protected. Malaria, waterborne illness, limited local healthcare, and mandatory vaccination requirements mean preparation is essential, not optional.

Mandatory Vaccination

Yellow Fever: International Certificate of Vaccination required for entry at Lungi Airport. No exceptions. Vaccine must be administered at least 10 days before entry. Keep the yellow booklet accessible — you will present it at immigration.

Strongly Recommended Vaccinations

Consult a travel health clinic at least 6 weeks before departure.

Malaria — The Primary Risk

Sierra Leone is in a high-transmission zone. Plasmodium falciparum (most dangerous species) is present year-round, peak transmission May-November. Risk applies to all visitors regardless of visit length.

Recommended antimalarials: Atovaquone/Proguanil (Malarone) — 1-2 days before, daily throughout, 7 days after leaving. Or Doxycycline — 2 days before, daily throughout, 4 weeks after leaving. Consult your doctor for the right choice based on your health history. Chloroquine-based prophylaxis is NOT recommended (widespread resistance).

Physical protection: DEET 30%+ on all exposed skin especially at dusk and dawn. Sleep under treated net. Long sleeves and trousers after sunset.

Medical Facilities

Freetown: Choithram Memorial Hospital (+232 22 223 330) — best private facility, used by expats and UN agencies. Connaught Hospital — government facility. Multiple private clinics throughout Aberdeen and Lumley.

Lungi: Basic health clinic in town. For anything beyond minor issues, Freetown is required. Hariom Yogi Guest House can assist with medical referrals and emergency transport.

Medical evacuation insurance is non-negotiable. Serious conditions cannot be treated in Sierra Leone. Evacuation to Ghana, Senegal, UK, or home costs USD 15,000-50,000+ without insurance. Comprehensive medevac coverage from ISOS, Global Rescue, or equivalent travel insurance is essential before departure.
